Perry and Daughter, Suzanna Perry, both of Raleigh, NC; Father, Otis Blaylock, Wyandanch, NY, Mother, Willie Mae Massenburg; Sister, Rita Harris; Brother, Prentiss Blaylock (Ladiner), all of Raleigh, NC; Preceded in death, Brother, Tony Blaylock; 2 Grandchildren, Taylor and Peyton; Nephews, Hericko Blaylock (Cheri) and Joshua Harris; Niece, Carmen Williams and Alexis Blaylock; Great-Nieces, Lilyan Grace and Jonnica (Joshua).
